CaP Program (see attached) has been developed by Gold Coast Primary Health Network (GCPHN) in partnership with Brisbane South PHN. A community-based initiative, CaP is designed to help medically vulnerable people and their support networks prepare for emergencies and severe weather events. The program supports participants to identify their health support needs, understand potential risks, and develop a simple, personalised preparedness plan to help them stay safe and connected during a disaster.
The workshop session is practical, easy to follow and focused on real-life situations. Participants are supported to think about questions such as:
What would happen if the power went out for several days?
How would I manage medications that need refrigeration?
Who could help me if my usual supports were unavailable?
What health information, equipment or supplies would I need if I had to leave home?
What simple actions can I take now to reduce my risk?
